Screening vs. Diagnostic Testing
Screening tests identify asymptomatic people who may have a condition so they can be evaluated further, whereas diagnostic tests confirm or rule out disease in people with symptoms or a positive screen.
Screening applies a test to an asymptomatic population to detect disease (or risk) earlier than it would otherwise present, with the aim of improving outcomes through early intervention. Diagnostic testing is applied to people who already have symptoms or a positive screening result, to establish whether disease is truly present. The same technology can serve both roles depending on context — a mammogram is screening for a symptom-free woman but diagnostic when evaluating a lump.
The distinction matters in cancer prevention because the two settings demand different test properties and have different stakes. Screening tests favor high sensitivity to avoid missing cases and must be safe, inexpensive, and acceptable for mass use; diagnostic tests favor high specificity and accuracy because they drive treatment decisions. The pre-test probability of disease is low in screening and higher in diagnosis, which strongly affects how a positive result should be interpreted.
A key pitfall is evaluating screening programs without accounting for lead-time bias (earlier detection can inflate apparent survival even with no real benefit) and length bias (screening preferentially catches slow-growing, better-prognosis cases). A screen that "finds cancer earlier" is not automatically beneficial; that requires evidence it reduces mortality or morbidity.
For example, a positive stool-based colorectal screen does not diagnose cancer — it flags the person for a diagnostic colonoscopy that confirms or excludes it.
